Short-Term Stay After Hospital Admission

Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

                    [review] => All I can say is run far far away from here. Made a huge mistake of selecting this place for my husband over a year ago only because they offered dialysis on premise. Well that didn’t last long as the dialysis company stopped performing their services at the facility a couple of months ago. I have a whole notebook of events at this place including an aide dropping him and it never being reported. My husband was 59 and quite alert at what happened. I had a nice rapport with a few nurses including the social worker but you go the next time and it’s new nurses constantly with nurses coming and going. Last straw was the house doctor taking my husband off of blood thinners when his neurologist was insistent on him being on them due to a stroke in 2022. In speaking with the doctor on 8/29 he said “oh he was bleeding” when he was on blood thinners (as he was fiddling on the computer trying to get his story right on what patient he was dealing with). Uhm bleeding was caused due to the fact that he picked a scab on his fistula.  He developed a blood clot in his fistula of his left arm where he had dialysis which killed his fistula and he had to have a port put in his chest to continue dialysis. Perhaps there wouldn’t be a blood clot had he been on blood thinners like he was supposed to be as he never had clot issues when on the baby aspirin. Well three days later after speaking to the doctor my honey died on September 1. Only took the house doctor 16 days to sign off on the cause of death even after the funeral home kept calling and sending many faxes to him and only took a threat of reporting him to the state that he finally signed it, all as my husband remained in a body bag for over 2 1/2 weeks at the funeral home until this was done. Place should be shut down and I will fight and make sure that nobody goes through what myself and our two children went through.

Update: today on 10/2 I finally received my husbands urn, necklaces etc. after a month. He’s home safely with me thank you lord. Dr. Rickie Sander you and I have only just begun introducing ourselves to each other and Angie thanks for scheduling my honey to go get his blood clot at the vascular center resolved, first on 8/20 and 8/30 only for it to somehow be cancelled and I have a voicemail of my honey the day before he passed on 9/1 saying how he was “screwed again” from getting this treated at the vascular center both of those times. Oh Lanier you don’t need to respond to the same repetitive response that you leave when getting bad reviews. Majority of the 5 star reviews are from the staff that works there (or used to work there) just to bring up the ratings. Like the saying goes “it ain’t over until the fat lady sings”

                    [review] => I would like to say thank you to all the nursing and therapy staff for all the love and care my grandmother received while in Lanier. While in hospital we were given a list of rehab centers and really was undecided. Roxanne in admissions called and asked if we would like to come and tour , or she could come to the hospital.
We met with her at UF Health and she was able to answer all of our questions . My grandmother was admitted and immediately started receiving excellent services. She was back home within a couple weeks. Thank you to the following...  200 hall nurses, caregivers, dietary , activities , Roxanne and Darlene

                    [review] => Please do not send your loved ones or seek treatment at this incompetent facility.  My cousin Edward Myers died in their care mysteriously and the family was not notified for several days after they sent my cousin to a funeral home that they send all of their deceased patients too.  Additionally, the family had to pay for an autopsy to be conducted and video footage of the night he mysteriously died is missing.  They need their license suspended immediately pending the outcome of a formal investigation by the State of Florida, Duval County and the City of Jacksonville.  I will be using all of my resources to get an answers and hold whoever is accountable for my cousin's untimely death.

                    [review] => I read all of these outstanding reviews and allowed my sister to stay at this place. For the life of me, I do NOT understand how they have good ratings. Maybe it's the employees. I have more complaints than time to post them, but let me say that, they do NOT communicate with the patient's families until it's time to collect money, and will bill you without your knowledge. My sister's home health PT/OT had her walking up 17 steps before going into Lanier and she came home not even being able to stand on her own, nor walk!!!! How is a patient's DECLINE rehab? Darlene Weston (social worker) also FAILED to send proper paperwork for a stay appeal and NEGLECTED to inform us that Lanier was charging my sister because of her act of INCOMPETENCE. She said to me that, my sister told her that she didn't want to be released, however when questioned in my sister's presence, my sister adamantly refuted this claim!!! Ms. Weston walked away. Then Ms. Weston had told me that MEDICAID was paying for my sister's stay, only to DENY that she had told me that when questioned in front of the business office staff! Lanier's staff's goal seemed to have been to transition my sister from SHORT-TERM to LONG-TERM and COLLECT the RETROACTIVE pay from medicaid, but because I told them to release my sister, they now want to bill her personally, even though we were NEVER INFORMED that she had reached her maximum medicaid stay. I have reported this UNETHICAL PRACTICE to the proper agencies and demand that Lanier waive the bill that is outstanding due to their NEGLIGENCE and NON-COMMUNICATION-- FROM THE UNPROFESSIONAL NURSES WHO DO NOT ACKNOWLEDGE YOUR PRESENCE TO THE ADMINISTRATIVE STAFF.

Elibox is a very rude and disrespectful worker and has no problem being disrespectful to the clients guest and even witnessed him being rude while I visit my grandmother. I would HIGHLY RECOMMEND HE NOT BE WORKING AT THE ESTABLISHMENT! He is a liability and he is not the kind of employee that you want to come across.                     [review] => I chose this place after reading the reviews over seven other locations in the area. It pains me to say that we have had the worst care virtually nonexistent from the staff at this location then I have ever experienced anywhere.

My mother was admitted 29 days ago and did not have a doctor visit until almost 2 weeks after she was admitted when we had to threaten taking her out of the facility. Once the doctor examined her and gave her a prescription for the medication she needed. They were not administered for another three days. They continue to skip giving her medication because they are careless. She has to prepare her own ice bags for her feet and using diapers and ice from the cafeteria. She has had an ear ache now for six days, on the fourth day after numerous upsetting phone calls the doctor finally came and checked her ear he ordered drops. But they still haven’t given them to her and now we are on day six. Six days of a bacterial infection and an elderly person is critical and could be life-threatening.

No one tries to call or call back when I call over and over again. They constantly lie about the care they are giving her and pass the buck to one another.

I am now trying to get her out of the facility and they are ignoring all request and coming up with things that they need us to provide. None of which we were told in the 29 days she has been there. I did have them call one time and ask for her savings account and checking account information which we quickly denied.

Not really sure what’s going on in this facility but I would definitely pick another one if you care about your loved one.

                    [review] => DO NOT send your loved one here. They are terrible, unsafe. Because of them, my mother's life is being cut short. They would not give her proper medication on time, and due to this neglect she got very very ill. They did not give her antibiotics on time whenever she was having a terrible infection. Due to this neglect, she has an infection in her heart valve that is irreversible. They lie and cover up each other's back. They refused to tell us any information as far as 2 weeks prior of what they were giving her, 2 weeks of notes and every thing they gave her and did. They did not give her physical therapy as they were supposed to. She has bed sores, she would have a foul odor as if they were not bathing her properly. My mother is terminally ill, and had they treated her properly instead of being negligent and ignoring her, she would have more time with us. Do not send your loved one to linear. They do not care. They don't care about medication, physical therapy, tlc, all they care about is covering each other's butts when mistakes are made. They have been giving my sister and I to run around about information we requested over a month ago. We will be getting a lawyer.

                    [review] => My stay at Lanier was horrible they lost my clothes they lied to me about home health it was just terrible the food was cold sometimes when I got it I mostly ate grilled cheese sandwiches and you can tell when the cooks wanted to make them and when they did that place was just horrible it's old and needs to be tore down and needs to be upgraded real bad three people and some of the rooms that place was nasty had a water main to break in the room that I was in and then had those big palmetto roaches that place was horrible I would not recommend this place to nobody's family no steady staff are mostly agencies staff nurses with attitudes the only best thing about this place is the rehab department they really try to help you get back on your feet and they need a bigger rehab place to do better work in but the rehab staff is fabulous that's the only thing good about that place lying social worker they have me believing I had home health and come to find out I didn't have no home health I had to do call for home health myself I'm handicap and disabled I'm paralyzed from the waist down I'm not able to do that I thought that's what social worker too help you set up for at home now I'm back in the hospital with an infection and I know they know that I had an infection Lanier never again

Quite a bit of money. He had 2 stimulus payments that didn't go to him but this facility had his card. He can't go anywhere. I switch nursing homes and his wounds are healing but Lanier still claims they don't have his card but SSA said someone they can't disclose to me is his payee. When I find out who, I will be pressing charges.

                    [review] => My mother was placed here for stroke recovery late Oct 2020 and died there Nov 10, 2020, alone. She could not speak, walk, ring a call button, but she went unmonitored for at least 3 hours before she was found deceased. I just received her things (given to a local family member at the time of her death). Her things were packed with no care, inside out, crumpled/balled up. Items were missing, at least one item that wasn't hers was in the bags. Her rosary was destroyed and just placed in multiple places all over 2 bags. If I had to do it all over again this would not be the place I would ever put her. No one got back to me after repeated attempts to learn of her condition. The director finally answered me but he didn't know more than her file which wasn't complete. But a patient who cannot speak or move should not be left for hours without someone checking on them.  I would never recommend this facility for anyone and medicare should consider removing them from their list of preferred faciities.

The staff members are attentive and they genuinely care about their residents. Furthermore, the activities department is remarkable and works hard to keep the residents engaged and entertained daily!                     [review] => Staff smiles alot, don't trust they are careing enough to help patients.  Aides dont change bedridden oatients more than once every shift. I was a rehab patient. Incontient wheelchair bound.  After 2 weeks, had to change myself.  Only had 2 showers in the month I was there, The staff didnt want to be bothered. I transfered myself into my own chair daily, the staff didn't know how to use a transfer board.  Don't allow your mom or dad to rot in any nurseing hone.  They are understaffed and still learning.

He was in terrible shape when he got there but was up walking around and cognitive again very quickly with the help of the wonderful staff.  Jojo, physical therapist, has a wonderful personality and Cynthia King, social worker, is excellent!  It was an overall great experience!  Would recommend.

                    [review] => My mother went to this place for inpatient rehab to recover from open heart surgery. They have both long term nursing home patients and rehab patients here with semi-private rooms separated by gender.  Although there are separate halls for nursing home patients, they have more nursing home patients than rehab patients, so they currently have some nursing home patients on rehab patient halls, but my mother had a very nice rehab patient roommate. My mother only stayed one night.  Although they knew that she was unable to use the restroom without assistance, it took about an hour for anyone to answer the call button during the night shift. They appear to be understaffed.  There is a huge difference between the number of cars in the parking lot during the day and at night.  Upon being confronted with the call button response issue, the facility administrator admitted that it should have taken no longer than 5-10 minutes for someone to have responded to the call button. My mother didn't feel safe there, so she left about 12 hours after arriving.

                    [review] => Lanier Terrace is your typical nursing home. MOST of the caregivers do not seem to care about their job, just about receiving a paycheck. I have found ants in my mother's bed. She was bitten by them. She is blind and crippled and could not get away from them. DCF was involved, but just as I suspected, NOTHING was done. I found ants in her room again since this happened. I would rather die than have to live at Lanier Terrace.

                    [review] => If you want your loved one to be cared for properly then do not put them in Lanier Manor. The nursing staff are rude and they leave patients/residents unattended for long periods of time, they leave them in soiled pampers and clothing for long periods of time, leaving the patients and residents with rashes. They administer meds without the consent of family members. Staff complains that this is a "thankless job" I dont know about you but if someone CHOOSES this line of work I believe they should have a passion for wanting to take care of the elderly whom can't take care of themselves and that in itself is reward enough. They are unprofessional and have been more worried with the asthetics of the place as opposed to the care they provide to the patients/residents. I do NOT recommend this nursing home.

I would like to say thank you to all the nursing and therapy staff for all the love and care my grandmother received while in Lanier. While in hospital we were given a list of rehab centers and really was undecided. Roxanne in admissions called and asked if we would like to come and tour , or she could come to the hospital. We met with her at UF Health and she was able to answer all of our questions . My grandmother was admitted and immediately started receiving excellent services. She was back home within a couple weeks. Thank you to the following... 200 hall nurses, caregivers, dietary , activities , Roxanne and Darlene
